    Summary Under provider orders the Medical Instrument Technician (MIT) functions as a member of the Cardiology multidisciplinary team. MIT at this level identify and record anomalies indicative of disease, injury, or other medically significant condition from ultrasound imaging and simultaneous recordings of the Doppler. They incorporate the causes and results of a variety of symptoms and conditions into a plan of ultrasonic diagnostic examination. Responsibilities The Echocardiography Medical Instrument Technician, GS-8, reports to the Outpatient Specialty Nurse Manager, who is the rating official; and to Nursing Services. Incumbent has experience to provide expert knowledge, skills, and abilities to conduct diagnostic echocardiography examinations in complex and unusual cases. Incumbent advises Cardiologist and/or referring physician of critically abnormal results of echocardiography studies, providing physician with accurate calculations and preliminary diagnostic evaluation of the echocardiogram. Produce diagnostic quality images and Doppler frequency recordings using M-mode, two-dimensional, and Doppler ultrasound. Administer contrast when required for image quality per protocol. Poised and articulate in communicating with people of varied educational levels and varied ethnic backgrounds. Must be willing to work cooperatively as a member of a team in all assignments, makes caring for the Veteran his/her priority while in the work setting, and demonstrates customer service principles in all aspects of work. Incumbent has knowledge of the anatomy and physiology of the heart and great vessels, knowledge of cardiac arrhythmias and their relationship to hemodynamic conditions of the cardiac cycle. Identify and record anomalies indicative of disease, injury or other medically significant condition from ultrasound imaging and simultaneous recordings of Doppler imaging. Intimate knowledge of thoracic anatomy and physiology related to cardiovascular function. Responsible for adhering to all departmental, service, hospital, agency, and national regulations and policies. Executes position responsibilities that demonstrate leadership, experience, and creative approaches to managing complex systems including the following. Knowledge of JCAHO and other regulatory requirements and assist with department regulatory compliance. Ability to communicate effectively and professionally with employees at varying grade levels. Conduct and oversee cardiac testing to determine the severity and variety of cardiac problems, i.e., valvular dysfunction, myocardial wall akinesia, effusions, endocardial lesions, etc. Perform complex procedures such as cardiac studies of the four chambers of the heart and valves using sector scanning techniques to detect stenotic and incompetent valves, chamber enlargement, and pericardial effusion and cardiovascular studies deriving pressure gradients across obstructed valves, etc. Set up and adjust the ultrasound equipment to meet the conditions of the examination and the patient and to operate the equipment for more complex, but standardized procedures. Perform complementary ultrasonic examinations (including noninvasive) transthoracic, Bubble studies and contrast echocardiography, (invasive) stress echocardiography, and transesophageal echocardiography. Perform IV insertion/discontinuation and to administer an ultrasound enhancing agent per local policy. Complete diagnostic studies ordered by, and in collaboration with, physician on scheduled tours of duty; coordinates appointments/workload according to appropriateness of patients/staffing. Manage echocardiography images in local PACs system and copy studies to CDs for Release of Information. Set up cardiac sonography equipment and ensures equipment functions properly. Expert ability to adapt transducer positioning, instrument controls, and examination techniques to the individual patient being studied and the problem being evaluated recognizing pertinent abnormalities and documenting abnormal findings.
