+ Intermediate skills in Microsoft Word, Excel, and Outlook required. Experience in PowerPoint and electronic medical record - NextGen preferred.

+ Must be 22 years of age or older with a valid Arizona driver’s license, and minimum of 3 years driving experience and meet requirements of Terros Health’s driving policy.

+ Must have a valid Level 1 Arizona Fingerprint Clearance card or the ability to apply for one apply for one within 7 working days of assuming role.

Qualifications

Chaplain Job Requirements:

+ Bacherlors degree from an accredited college or university in Biblical or Religious counseling/studies

+ At least one year of related experience, with a knowledge of psychiatric patient care techniques with understanding of mental illness and chemical dependency (12 -step program), preferred.

+ Must have a valid driver's license and agree to a Motor Vehicle Driving Record check as part of the background check. And that record must be in good standing.

+ Completion of BLS & First Aid Certification within 30 days of hire.

Full-Time Family Nurse Practitioner Needed at our Priest Drive Recovery Health Center location in Tempe, AZ.

40 hours/week

Schedule: 4-10's; Tuesday-Friday, 7am-5:30pm

Current schedule 30-40 hours at Priest; Possible 1 day at a different east valley clinic

(Preferred Candidate = At Least 2 Years of FNP Experience Preferred)

Collaborate with the Multidisciplinary Team and Mental Health Provider on site to provide integrated care

Candidates Must Be AZ Licensed with a valid DEA

PCP provides direct primary health care to assigned patients

PCP Performs independently or as a part of a treatment team

PCP will maintain appropriate knowledge, skill and certification for this position

PCP provides direct patient services not less than 85% of the work time

PCP employs therapeutic interventions appropriate for identified treatment issues

PCP seeks appropriate clinical supervision/support, when necessary, to ensure the provision of effective treatment

PCP establishes appropriate patient files that conform to agency guidelines, best professional practice and State licensing regulations

The ACT Employment Specialist engages members and provides direct treatment, rehabilitative and supportive services in a community setting. Works as a team member of a clinical team to assess member needs and wants, participates in the development of individual service plans, acts as a patient advocate and liaisons with families and community agencies. Provides on-call crisis intervention services, as well as weekend and holiday coverage.

The ACT Employment Specialist duties include:

+ Facilitates educational and supportive activities including orientation into the work environments, the effect of work and meaningful community activities on benefits programs, job seeking skills, benefits counseling, job clubs, and employment support groups.

+ Works as a team member of a clinical team to assess member needs and wants, participates in the development of individual service plans, acts as a client advocate and liaisons with families and community agencies.

+ Serves as the team’s specialist for employment services by collaborating with other team members in the assessing for and delivery of employment skills training as specified in the individual service plan.

+ Provides on-call crisis intervention services, as well as weekend and holiday coverage.

Qualifications

+ Bachelor's degree in Behavioral Health or related field or a combination of experience and education.

+ Three years experience in the delivery of behavioral health services required.

+ A minimum of one year of vocational rehabilitation training or supervised experience as a Rehabilitation or Employment Specialist, or Vocational Rehabilitation Counselor required.

+ Three years working as an employment specialist preferred.

+ Interpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community members.

+ Good communication skills, evidenced by a non-judgmental approach, effective listening and positive interactions.

+ Basic understanding of the DSM-V.
