    Education: Graduate of accredited School of Nursing or baccalaureate program required; Current licensure as a Registered nurse thorough the State of Arizona.

    Experience: Clinical experience in a psychiatric setting preferred.

    **Position Profile**

    Resident Care Team (RCT) RN case managers have the primary responsibility to provide and coordinate hospice care for patients living in Resident Care Facilities and to work closely with the facility's staff. They deliver care and oversee the interdisciplinary hospice team to ensure patients' physical, emotional and spiritual needs are met. RN case managers educate patients and families about the disease process, help manage and control symptoms and offer support and strength during difficult times.

The RN has knowledge of professional nursing principles, practices, concepts, and procedures as applied to the care of the patient population served.
