Qualifications

Job Requirements:

+ Current license to practice nursing in Arizona or compact privileges.

+ Preferred one year of experience working in a psychiatric inpatient setting with patients aged 12-65yrs+. **For our geriatric unit, 1 year of med/surge experience is strongly preferred.

+ Must have or able to obtain a Level 1 Fingerprint Clearance Card.

Qualifications

Education: Graduate of accredited School of Nursing or baccalaureate program required; Current licensure as a Registered nurse thorough the State of Arizona.

Experience: Clinical experience in a psychiatric setting preferred.

MINIMUM REQUIREMENTS : Must be certified in CPR upon hire and bi-annually thereafter and must possess or obtain and be able to perform Handle With Care within 30 days of hire and re-certify as needed. Must complete all required mandatory in-services annually. Must possess an Arizona Fingerprint Clearance Card or the ability to obtain. Must be tested for Tuberculosis with a PPD skin test or chest x-ray upon hire and as required thereafter.

The Clinical Sales Liaison, RN is responsible for patient education and sales in a hospital setting. This Clinical Sales Liaison, RN will work directly with case managers and discharge planners to process home infusion referrals within the hospital systems. This position requires sales training as directed by Regional Sales Director.

**Primary Responsibilities:**

+ Provide clinical education and manage marketing services to hospital and physician referral sources to ensure awareness of Optum Specialty Pharmacy Infusion Services programs and services

The Quality Program Manager for the Patient Navigator program will be directly responsible for the day to day operations of a virtual call center, serving all care delivery organizations across Optum Care. As a leader on the Optum Clinical Performance Quality team, you will be part of UnitedHealth Group's mission of helping people live healthier lives.

**Primary Responsibilities:**

+ Accountability for the business operational functions of a 20+ associate call center focused on assessment, education, and support for the patients of our care delivery organizations (CDOs) in Optum Care

+ Serves as a supervisor and role model for the patient navigator team and mentors new employees to the team as necessary. Provide direction, development, and support to supervisor level associates.

+ Oversees the implementation of new CDO clients to the program inclusive of developing care transition processes and community resource directories specific to each market, in partnership with multiple teams in the CDO including quality, clinical services, and service centers

+ Works collaboratively with internal quality leaders from the Member Experience (CAHPS/HOS) and performance pod management teams to manage the CDO client relationship

+ Works collaboratively with payer program leaders and teams to create an integrated experience for the patient

+ Serve as a subject matter expert for CAHPS/HOS quality survey measures and Medicare Star Ratings for the Patient Navigators and for CDO clients

+ Create and maintain monthly dashboard/report for each business supported providing a status of key program compliance indicators for CAHPS/HOS

+ Adapts departmental plans and priorities to address business and operational challenges

+ Accountable for uniform and consistent application of clinical processes / workflows within team and books of business, as well as effective implementation and management of all workflows with CDO and payer

+ Provide subject matter expertise, coaching, and training to other Optum teams implementing similar programs using contracted resources, as needed

+ Support program growth and efficiency through participation in operational projects to drive efficiency such as selection of a call center documentation platform, dialer systems, and additional communication methods including IVR, text, and email

+ Support organizational growth by evolving the management structure for the Patient Navigator team, which is expected to grow year over year

+ Active RN license

The Integrated Care Team leverages a cross-functional approach, supporting the member end to end, with close collaboration and co-management. Generally, work is self-directed and not prescribed; is able to work with less structured and more complex issues.

The Preservice Review/Prior Authorization RN will manage the full array of health care services, spanning both outpatient and Inpatient settings and with support from some specialized clinical teams, serves as a resource to others.

This new cross-functional collaboration provides an opportunity to deliver the next generation clinical model, integrating services from precertification, inpatient concurrent review, post-acute review, discharge planning, medical and behavioral care management and prevention to improve quality of care and patient outcomes. Candidates for this position would be required to support cross-training to multiple roles: Prior Authorization (PA), Post-Acute Care Management (PACM), Inpatient Care Management (ICM) and Discharge Care Management (DCM) to support fluctuating inventory within the Integrated Care Team.

+ Perform initial and ongoing reviews pertaining to primary role (PA, ICM, DCM, PACM), appropriately applying clinical criteria (InterQual, policies, National and Local Coverage Determinations) for approval and sending to Medical Directors if review is necessary for determining an adverse determination

+ Collaborate with Medical Directors, clinical peers and Health Plan partners on performing appropriate clinical decision making and/or referrals

+ Apply clinical expertise when discussing case with internal and external Case Managers and Physicians

+ Follow all Standard Operating Procedures in end-to-end management of cases

+ Participate in team meetings, education discussions and related activities

+ Maintain compliance with Federal, State and accreditation organizations

Qualifications

As a Telephonic RN Case Manager/Disease Manager, you'll wear many hats, and work in a variety of environments. Sometimes, you'll interact with members leaving the hospital - possibly with new medications or diagnoses. You may also act as an intermediary between providers and members - serving in numerous roles, such as educator, evaluator, service coordinator, community resource researcher and more. The result? **Primary Responsibilities:**

+ Provide a complete continuum of quality care through close communication with members via phone or video platform interaction

+ Support members with diabetes, kidney disease, prediabetes and / or asthma condition education, medication reviews and connections to resources

+ Assist members recently discharged from the hospital or Emergency Room learn best disease management self-care practices

**Required Qualifications:**

+ Current, unrestricted RN license in state of residence

+ Willing and able to obtain a NV state nursing license

+ Proven bilingual Spanish

+ Proven problem solving skills; the ability to systematically analyze problems, draw relevant conclusions and devise appropriate courses of action

+ Willing and able to work some evenings and every Saturday

**Preferred Qualifications:**

+ Bachelor of Science in Nursing (BSN)
