    POSITION SUMMARY

    The Care Navigator, who can be any person with medical experience (Medical Assistant, Certified Nursing Assistant, Emergency Medical Technician, etc.) will meet with members in their homes to complete preventative screenings. As a Care Navigator, you will work with and within physician practices to review medical charts with the goal of capturing missing data that will support care gap closures. In the Care Navigator role, you will also communicate with patients regularly to obtain current statuses, review visit plans, provide education, and collaborate with other professional staff. Additionally, you will coordinate appointments and other service needs for a higher rate of care gap closure while meeting the outlined quality measurements.

    This position requires travel to Benson, around Cochise County, and surrounding areas to provide in-home support to our patients.

    DUTIES & RESPONSIBILITIES

    Work collaboratively within a team setting to provide positive patient outcomes.
    Demonstrate professional, appropriate, and effective communication with patients, families, and other team members to ensure the continuum of care.
    Maintain patient confidentiality, following HIPPA guidelines and regulations.

    6 Explains informed consent to applicant donors. This includes explanation of procedures, hazards and potential adverse events; explanation of immunization schedules, dose and antibody response; providing clear opportunity for donor to refuse participation; obtaining signature and date certifying informed consent.

    7 Handles medical emergencies at the Center by providing routine care as outlined in Standard Operating Procedures (SOPs); elevates situations to Center Physician or local medical service providers as required. Periodically checks emergency kits to assure supplies are present and in-date.

    8 Reviews test results and determines continued donor suitability. Follows SOPs regarding acceptable ranges of four-month required test (PE/STS) reports. Maintains knowledge of and apply all rules regarding four-month approvals and grace periods.

    9 Assess signs and symptoms a donor may exhibit that may result in the inability to continue donating.

    10 Manages employee hepatitis B and influenza immunization program, including administration of immunizations.

    11 Understands the policies and procedures associated with hyper immune programs at the center where applicable, and oversees donor immunization programs. Reviews antibody titer results as required. Maintains awareness of appropriate immunization schedule for each antigen.

    12 Ensures the accurate recording of donor data in the electronic donor information management system as outlined in the SOPs.

    13 Maintains clean efficient work environment, and ensures sufficient operating supplies and forms are available as needed. Follows all Health Safety & Environmental (HSE) and Occupational Safety and Health Administration (OSHA) policies and procedures. Promotes safety in all actions. We are an Arizona Department of Health Services-accredited Cardiac Receiving Center and a Nurses Improving Care for Health system Elders-designated senior-friendly hospital. The hospital is staffed by physicians who are full-time faculty of the University of Arizona College of Medicine - Tucson and is managed by Banner Health under an operating agreement with Pima County. Our specialty services include inpatient and outpatient behavioral health, treatment and education for diabetes, innovative geriatrics care and comprehensive orthopedics.

    This position is responsible for patient care in the emergency department as assigned under the medical direction of the base hospital EMS Medical Director. This position assesses, plans, implements, evaluates and documents care of patients in accordance with organizational policies and in accordance with standards of professional paramedic practice. This position is accountable for the quality of patient services delivered by self or others who are under their direction. This position utilizes specialized knowledge, judgment, and skills necessary to assess data and plan, provide and evaluate care appropriate to the physical and developmental age of assigned patients.

    CORE FUNCTIONS

    1. Evaluates acuity level of patients and insures appropriate matching of patients care needs to unit and hospital resources. Demonstrates the ability to assess data reflective of the patient’s status and interpret the appropriate information needed to identify each patient’s requirements relative to their specific needs.

    2. Performs basic patient care appropriately within scope of training: positioning, transportation, phlebotomy, ECGs, maintenance of spinal precautions. Administers or monitors medications when indicated and as ordered by a physician. Such medications are limited to those approved for administration and/or monitoring by a paramedic in the State of practice. Assists patient in transfer using proper body mechanics and transfer techniques. Starts peripheral intravenous lines, including external jugular lines, initiates infusion, and maintains and adjusts fluid administration per physician order.

    3. Performs cardio-vascular monitoring and management within scope of training.

    4. Performs airway management within scope of training and within state guidelines to include oxygen administration, tracheal suctioning, assisting with ventilation, positioning, end-tidal CO2 and pulse-ox monitoring.

    5. Actively participates in the resuscitation of critical patients as assigned. Observes and monitors patients through the diagnostic phase of treatment.

    6. Performs other duties as assigned based on competency and training.

    7. Recognizes changes in patient status that requires notification of charge or primary nurse and physician. Keeps charge or primary nurse informed of patient status and current activities. Alerts responsible physician to changes in patient status.

    8. Documents assessment and reassessments within unit guidelines on appropriate documentation forms. Documents the incorporation of assessment findings and appropriate standards statement to support desired Patient Outcomes. Documents patient progress toward outcomes and responses to interventions/therapeutic modalities example: Response to pain medication. Documents the discharge plan of care and services consulted during patient visit.

    9. Interacts with all levels of staff in a variety of departments, physicians, patients, families and external contacts, such as employees of other health care institutions, community providers and agencies, concerning the health care of the patient. Interacts with other health care providers in numerous settings in order to report and ask for or clarify information.

    MINIMUM QUALIFICATIONS

    High school diploma/GED or equivalent working knowledge.

    Must possess a current and valid Paramedic certification or license within the state of practice and maintenance thereafter. Current BLS and ACLS certification is required. PALS is required if working in the Emergency Department or with Pediatric population. Additional certification or continuing education may be required based on specialty of practice.

    Moderate experience typically gained through 2-4 years relevant work experience OR 12-18 months in an educational program.

    PREFERRED QUALIFICATIONS

    FP-C or CCT-P Certification if working in ICU, Burn or CVCU.
