    + Following the plan of care, instructs and aids patients in active and passive exercise, muscle re-education, gait, functional, ADL, transfer, safety, and prosthetic training.

    + Observes, records, and reports to the supervising PT, the supervising nurse, and/or the physician the patient's response to treatment and changes in the patient's condition.

    + Instructs patient, family, caregiver, and other members of the health care team in the areas of therapy in which they can participate to assist the patient.

    + Visits patient according to Plan of Treatment; complete a progress note for each visit and submits progress notes per LHC policy.

    + Participates in developing the Plan of Treatment.

    + Participates in staff conferences and committees as necessary.

    + Adheres to LHC Therapy Standards of Practice in provision of services to all patients.

    Education & Experience

    License Requirements

    + Current licensed, registered, or certified to assist in the practice of physical therapy in state of practice.

    + Current CPR certification is required.

    + Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.

    Additional State Requirements

    + AZ: Must have 2,000 hours of experience under onsite supervision post certification prior to working in home health.

    Although licensed as long term care, Banner Boswell’s Skilled Nursing Facility (SNF) is focused on short term rehab with the average length of stay for each patient being 12 days. Banner Boswell’s SNF is a 79-bed facility that manages patients with neurological, orthopedic and complex medical diagnoses and illnesses that prevent these patients from returning home immediately from the hospital. As a **Physical Therapist Assistant** you will work in collaboration with strong case management and nursing teams and therapists to help patients meet their goals and establish safe discharge plans in an effort to prevent readmission to the hospital. The average daily caseload for each therapist is 6.5 hours of patient care in an 8 hour shift (7-13 patients on each therapists schedule per day) and the average length of stay is actually 11 days. This manageable caseload allows each therapist to spend the needed time truly getting to know each of their patients in order to build the rapport and trust needed to help each patient meet their goals. All PT Assistant’s are responsible for documenting all treatment in patient charts and completing all written communications in a timely manner, i.e., progress notes, discharge summaries, daily S.O.A.P. notes, charge slips, and daily total sheets, and insurance/plan of care forms as necessary. Physical Therapist Assistants are also responsible for keeping the patients chart up to date with current scripts, tally sheets, and monitoring insurance coverage.

    + Establish and develop professional relations with other potential referral sources including:

    + Physician contacts – inform physicians of our services, locations, quality patient care and philosophy to be the best provider of therapy services in the area working

    + Attend patient physician appointments and perform shadow opportunities when possible

    + Provide appropriate and timely written and/or verbal communication and follow-up when necessary

    + Continue the development of professional skills and interest through continuing education course, program involvement, staff in-services and professional meetings

    QUALIFICATIONS:

    + Current licensure as a Physical Therapist Assistant in the state practicing

    + Associates degree, accredited by an accredited PTA Program

    + Current CPR certification and AED training

    + Minimum of 2 years experience outpatient experience preferred

    + Strong communication skills

    + Observes, records, and reports to the supervising PT, the supervising nurse, and/or the physician the patient's response to treatment and changes in the patient's condition.

    + Instructs patient, family, caregiver, and other members of the health care team in the areas of therapy in which they can participate to assist the patient.

    + Visits patient according to Plan of Treatment; complete a progress note for each visit and submits progress notes per LHC policy.

    + Participates in developing the Plan of Treatment.

    + Participates in staff conferences and committees as necessary.

    + Adheres to LHC Therapy Standards of Practice in provision of services to all patients.

    Education & Experience

    License Requirements

    + Current licensed, registered, or certified to assist in the practice of physical therapy in state of practice.

    + Current CPR certification is required.

    + Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.

    Additional State Requirements

    + AZ: Must have 2,000 hours of experience under onsite supervision post certification prior to working in home health.
