Requirements to include:

Must be licensed as a Speech Language Pathologist (SLP) in Arizona
Must possess or be able to obtain a Level 1 Arizona Fingerprint Clearance Card
Must have a Valid Driver's License
Must have reliable transportation to travel to members homes or community locations for ongoing treatment based on members Certified Plan of Care
Bilingual (English/Spanish) is a plus
Ability to provide required direct/indirect supervision of Speech Language Pathology Assistants (SLPA's) is preferred +
Duties to include:

Evaluate members for Speech Therapy services
Complete Re-evaluations for services as needed
Develop and Update Individualized Certified Plan of Care focused on clients reaching their full potential
Provide ongoing treatment to clients based on their Certified Plan of Care in their home or community locations
Complete SOAP notes electronically
Complete progress reports electronically
Effectively communicate with clients, families, assistants, etc.
Excellent written and verbal communication skills
Supervision of Speech Language Pathology Assistants (SLPA's) +
Review quarterly progress reports +
Review SOAP notes +
