Requirements:
Must be a Certified Occupational Therapy Assistant (COTA) in Arizona
Must possess or be able to obtain a Level 1 Arizona Fingerprint Clearance Card
Must have a valid drivers license
Must have reliable transportation to travel to members homes or community locations to provide ongoing treatment based on the members Certified Plan of Care
Must be willing to provide services via teletherapy based on members preferences
Duties
Provide reliable, ongoing treatment for members based on their Certified Plan of Care
Document Client Progress electronically
Collaborate with Supervisors and other therapists involved with the members case
Educate and train parents and caregivers on how to support the member with their goals at home and in the community
Effectively communicate with clients, families, supervisors, etc.
Excellent written and verbal communication skills are necessary
Must be a self-starter
Spanish speaker is a plus
