OUR NAME SAYS IT ALL… TRANSITIONS mission is to enable those with developmental disabilities to transition into a fuller, more independent life and integrate into the community. Our organization provides the highest quality care for people with disabilities to teach them the necessary life skills to learn, grow, and enrich their lives. Our quality services and supports are designed to empower the individual to participate more fully in all aspects of daily life. By promoting choice as well as individualization, our clients learn to grow and gain valuable life skills to live more independently in the world. Our trained, experienced, and certified Staff and Support Team will assist individuals in every possible way to lead fuller, happier lives and reach their highest potential. TRANSITIONS for the Developmentally Disabled serves Individuals with Special Needs who receive services through the Division of Developmental Disabilities – DDD. If your loved one does not receive services through DDD, TRANSITIONS also accepts private pay consumers.



TRANSITIONS provides Home & Community-Based Services (HCBS) which include Respite, Habilitation and Attendant Care for Individuals with Developmental Disabilities. These services provide temporary relief for the primary caregivers, one-on-one life skills training and assistance in maintaining a safe and healthy living condition. TRANSITIONS also provides physical, mental, and emotional support, training, and supervision for individuals moving into their own customized living arrangement. We offer the highest quality supervised HAI / IDLA homes for individuals. After graduation, TRANSITIONS offers a Day Treatment and Training for Adults (DTA) program which runs Monday – Friday.

Our ideal candidate is compassionate, patient, self-motivated and reliable. Our members often need a little extra encouragement to complete tasks independently. It is beneficial to have an employee that can be creative in teaching skills to help increase independence, without doing the task for the member. Consistency is key, while being creative in teaching a skill is great, we also need someone that can be consistent to help our members maintain skills they already have. We would prefer the candidate have experience working with individuals with developmental disabilities, whether it be professional or personal experience with a loved one, but it's not required.
