Position Summary
Serves persons with mental illness as part of treatment, in a 24/7/365 Assisted Group Living Treatment Facility (a home-like setting) with a support team, providing protective oversight, behavior management, personal support, assistance with activities of daily living, assistance with self-administration of medication, personal care services as required, and social and recreational activity consistent with the Client's Individual Service Plan. Provides care to clients in a single-family residence, during an assigned shift ensuring clients are never left alone in the facility. This position is classified as Safety Sensitive.
This position does not have direct reports.
Essential Functions:
Care Delivery
Carries out all duties in accordance with all program guidelines for care delivery as outlined in the Client's Individual Service Plan
Promotes the independence of all program clients in all activities
Promotes the dignity of clients by working with them in a positive, respectful, age appropriate and courteous manner
Provides therapeutic interactions with client in a way consistent with guidelines and directions contained in the Client's Individual Service Plan
Respects the privacy of the clients' personal space and property
Maintains confidentiality of client information
Assists clients with the self-administration of medication according to FSL-AGL Policy and Procedures
Assists with all activities of daily living, toileting and personal grooming as required
Prepares and assists with the preparation of meals and kitchen clean up
Conduct housekeeping activities as scheduled or directed
Shops for food and supplies for the household as required by management
Escorts or accompanies clients to medical appointments and other activities outside the home
Maintains documentation concerning clients and home operation as required
Attends staff meetings for the home and training as required
Assists in problem solving with Manager, BHT and other staff members as required
Attends clients’ staffings as needed providing input as a member of the service planning team
Contributes to meeting the requirement for an hour per week of clinical oversight as needed
Quality & Compliance
Participates in the Performance and Quality Improvement initiative at the agency and program level by learning about the initiative, offering input, suggesting performance improvements, and performing other PQI duties as requested
Attends mandatory and elective training during the first year of employment and complete required hours each year after the first

Requirements
Education/Experience/Credentials/Knowledge/Understanding
High school diploma or General Education Diploma
Minimum of two weeks behavioral health experience
Knowledge and understanding of the needs of vulnerable adults such as seniors and those with disabilities
Clear fingerprint background screening through the Department of Public Safety and comprehensive background clearance
Fingerprint Clearance card must be in possession, prior to starting
Meet all state and federal immunization and communicable disease clearance requirements with or without an accommodation or exemption
Must successfully complete Certified Caregiver Certification within 30 days of employment.
Must be at least 18 years of age
